This registration form is only for RESTAURANTS that have a physical storefront address on Main Street North between Manitoba Street and Ross Street.

Registration Package Options

Main Street Silver Package: 15x15 space.

Main Street Gold Package: 15x20, Collectable Lanyard/swag bag, discounted rates for additional footage (up to 10feet), social media shout out

Trade Options: Once again we will are looking to trade food vouchers (Valued at $15/voucher and provided by Sidewalk Days) for up to half of your pre-tax registration cost. Limited availability.

After submitting your application Trish our Vendor Coordinator will issue you an invoice and details about how to make payment. Please allow up to 3 days for her to respond with the electronic invoice.

Menu Style

List of Planned Menu Items

1. Name of Premise(s) where foods will be prepared

2. What type of containers will be used to transport foods to the Event?

3. List type and amount of cold holding units. (E.g. refrigeration unit, coolers freezers)

4. List type and amount of cooking and/or reheating equipment.

5. List type and amount of hot-holding units that will be used to keep hot foods hot.

6. How will food temperatures be monitored during the event?

7. Describe the number, location and set-up of hand washing facilities to be used by Event workers.

8. Describe where and how dishwashing will take place.

9. List the type of sanitizer to be used on food contact surfaces (bleach or quat). Will test strips be provided?

10. How will waste water, from dishwashing and hand washing, be collected and disposed of?

11. Washrooms are to be provided for food service workers. Where are they located?

12. Services required (Gas, Propane, Electricity)

13. Where will your water come from? Where will it be disposed of?

Staff - Safe Food Handling Course List names of staff that have completed the Safe Food Handler’s Course

Staff - Temperature Logs List name of staff on each shift responsible for taking food temperatures and writing results down on paper.

Volunteer Appreciation Give Back

Sidewalk Days wouldn't be successful without the volunteers who help set up, supervise the children's village and act as ambassadors for the city. We are asking you as a vendor to help show them how much they are appreciated by donating a gift certificate or tangible item from your business.

Good Neighbour Acknowledgement

I have read and reviewed the Vendor FAQ and understand the rules and requirements for the use of generators and speakers/stereos. I understand that my booth may be shut down by the Moose Jaw Kinsmen Sidewalk Days Committee if I fail to comply with those requirements and that no refund will be received.

Vendors can cancel their registration until June 1st 2025 and receive a refund less a $50 administration charge, Cancellations after that date and/or event no shows are not eligible for any form of refund.
